Skilled Swing Bed
Skilled Swing Bed Program
When you or a loved one needs extended skilled care following a hospital stay, Patterson Health Center offers the solution with our Skilled Swing Bed Program. Located close to home, you'll receive compassionate care from a team of familiar, local professionals.
Why Choose Us?
Even if you've received initial surgical or hospital care elsewhere, you have options! You can transfer to Patterson for your recovery and rehabilitation in our Medicare-approved Skilled Swing Bed Program.
How our program differs from most skilled nursing facilities:
- Advanced Registered Nurse Practitioners (ARNPs) oversee care with at least two visits weekly.
- Low nurse-to-patient ratio.
- All the benefits of the hospital, including the lab, medical imaging, and pharmacy.
- Physical and Occupational therapy are tailored to your needs, and Speech Therapy is available up to 3 days per week to speed recovery.
- Complex wound care with a certified wound care nurse.
- Specialists are available for consultation, including rehabilitation, cardiology, and behavioral health.
- PHC does not provide long-term care; our success is your safe discharge home.

Frequently Asked Questions
Does Medicare cover Swing Bed care?
If you have Traditional Medicare insurance, it will cover 100% of Medicare-covered charges for the first 20 days as long as you meet Swing Bed criteria. For days 21 through 100, there is a daily coinsurance rate set by Medicare that may change annually.
If you have a Medicare Advantage or commercial plan, please check your policy for skilled nursing facility benefits, or reach out to our Case Manager. We would be happy to review your coverage with you!
What services are included in my stay?
Medicare-Covered Services include a semi-private room, meals, skilled nursing care, physical and occupational therapy, speech therapy, respiratory therapy, medical social services, and medications.
What should I pack for my stay?
We recommend bringing 3-4 outfits, plus sleepwear. You will also need a sturdy pair of shoes for exercises, as well as slippers or other lounging footwear. Please also bring any home medical equipment you regularly use, such as a CPAP, BiPAP, walker, or wheelchair. PHC provides needed hygiene items, but you may bring your personal hygiene items for your comfort.
